Editing an Existing Audit Form


To edit an existing audit form, click the EDIT pencil beside the form you want to edit. If no results from audits conducted on this audit form have been uploaded to the system yet, you may edit any of the data associated with the form.

However, only certain modifications can be made to an audit form if the results from an audit conducted on the form have already been uploaded into the system. This is because the results of all audits conducted on a specific form must contain the same information in order to create a meaningful report. Therefore, you may change the Audit Name or Audit ID, for example, but you may not add another question or change the point value of a question once an audit has been conducted on that form.

Once an audit has been conducted on a form and uploaded into the system, you may only edit the following information:

  • Audit ID and Audit Name
  • Active/Inactive status
  • Description
  • Question text *
  • Audit Category Name
  • Answer text *
  • Audit Category View
  • Comment text
  • Answer All option
  • Ratings file
  • Order in which questions appear
  • Instructions file
  • Order in which categories appear
  • Feedback Reports
  • Order in which choices (answers) or conditions appear
  • Configuration settings
 
  • Users, Roles, Locations, and Management levels associated with the audit form.
*Use extreme care when changing the text of a question or an answer choice to prevent invalidating reports based on specific audit results. For example, you may want to re-state the question to make it easier for the auditor to understand, but if you change the question altogether, the audit results may become meaningless.

Once an audit has been conducted on a form and uploaded into the system, you may NOT edit any of the following information:

  • Category ID
  • Points earned for an answer
  • Question ID
  • Total points possible
  • Question type
  • Default status
  • Audit type
  • Critical status
  • Force flag
  • You may not add questions
  • Whether or not the question must be answered
  • You may not add comments
  • Whether or not this is a CORRECT, INCORRECT, or INFORMATIONAL answer
  • You may not add audit categories or subcategories
  • Whether or not this answer choice requires a signature
  • You may not add report categories or subcategories
